Auscare Support coordinates aged care through the Support at Home program, in partnership with Trilogy Care.
We find the right care workers and services, and make sure things run properly on the ground.

Two proud moments. One incredible community. 💚

At Auscare Support, we believe meaningful change happens when people come together, share their experiences, and create opportunities for connection.

That's why we're proud to celebrate two special moments with the upcoming Inner West Disability Forum in September.

Our Business Development & Innovation Manager Susan Carter representing Auscare Support for the first time as a new member of the Inner West Disability Forum Committee, and our very own Lachelle Uzcateguigaymon Support Coordinator, who has been announced as a speaker for this next orum, sharing her insights as part of a conversation focused on mental health, wellbeing, community connection, and lived experience.

Rennee's story, continued.
Rennee brings more than training to her coordination work. She brings years of experience on the other side of the table, navigating plans, fighting for supports, learning the system the hard way.
That's why she coordinates the way she does: with the patience of someone who's been the person waiting for a call back, and the clarity of someone who now knows how to get things moving.

How our partnership with Trilogy Care actually works.

Together we cover the whole journey, so families can focus on what matters most: living well at home. Auscare Support brings coordination and connection. We source care workers and services that are the right fit for each person, and make sure everything runs smoothly on the ground.

Trilogy Care manages the Support at Home plan behind the scenes. Ongoing support planning, a dedicated care partner, funding management including invoice payments and reimbursements, compliance and background checks on all service providers, and access to experienced clinical nursing support.
