Protein powder: whey, collagen or plant-based? What's the difference?

Not all protein powders are nutritionally equivalent, and the best choice depends on what you’re using it for.

Whey protein is dairy-derived and contains all nine essential amino acids, including a relatively high amount of leucine. Leucine plays an important role in signalling muscle protein synthesis through the mTOR pathway, which is why protein quality matters when maintaining muscle is a priority.

Collagen protein is quite different. It is usually derived from bovine or marine sources and is rich in glycine and proline, but contains much less leucine and fewer essential amino acids than whey. Collagen has other potential uses, but it shouldn’t be considered equivalent to whey when the goal is supporting muscle protein synthesis.

Plant proteins, such as pea or soy, can be excellent options for people following a vegetarian or vegan diet. Some products combine different plant proteins to provide a broader essential amino acid profile.

So what should you look for?
• The type and source of protein
• Protein in grams per serve
• Essential amino acid profile
• Leucine content
• Added sugars and unnecessary ingredients
• Whether it suits your dietary requirements and preferences

For muscle health, around 2–3 g of leucine per meal is a useful practical target, although there isn’t one exact threshold that applies to everyone.

And remember, protein powder is a supplement, not a requirement. Whole foods should form the foundation of your protein intake where possible.

Muscle mass is only part of the story. How do we measure how well your muscles work?

When we monitor muscle health during weight loss, we look beyond body composition. Muscle function is also important.

Two simple tests we can use are the hand grip strength test and the sit-to-stand test.

A hand grip dynamometer measures how strongly you can squeeze. It gives us an objective measure of muscle strength.

The sit-to-stand test looks at functional strength. It measures how easily and quickly you can repeatedly stand up from a chair using your legs, without using your arms.

Together, these tests give us information about how your muscles are performing, not just how much muscle you have.

This matters during weight loss because changes in muscle mass and muscle function don't always happen at the same rate. Someone can maintain a reasonable amount of muscle mass but still experience a decline in strength or physical function, or visa versa.

By looking at both muscle mass and muscle function, we can get a much clearer picture of your muscle health during weight loss.

Healthy weight loss isn't just about losing weight. It's about preserving the strength and function that help you to live well.

Three years ago today, we welcomed our very first patient.

What began on 10 July 2023 with a simple vision - to provide evidence-based, compassionate and respectful care for people living with overweight, obesity and metabolic disease - has grown into something truly special.

Over the past three years, we've had the privilege of caring for well over a thousand patients and celebrating countless moments of improved health, confidence and quality of life. Thank you to every patient who has trusted us to be part of your journey.

Today, MedSurg Weight Loss is proud to be home to a multidisciplinary team of five special interest GPs, two experienced dietitians (including a dual-trained Diabetes Educator), an exercise physiologist, psychologist, and psychiatrist. Together, we're committed to delivering comprehensive, whole-person care under one roof.

This year has been an exciting one, with the expansion and renovation of our clinic, allowing us to welcome new clinicians and broaden our services to include women's health, menopause care, men's health and ADHD care alongside our expertise in obesity medicine and metabolic health.

Beyond the clinic, we're passionate about changing the conversation around obesity. Through community education and healthcare professional training, we're committed to reducing stigma, improving understanding of obesity as a chronic disease, and helping more Australians access respectful, evidence-based care.

A new oral GLP-1 medication has just launched in the United States.

For many patients, this represents another important step forward in the treatment of obesity and metabolic disease.

Unlike the injectable GLP-1 medications many people are familiar with, this new therapy is a tablet taken once daily. It works on the same appetite and satiety pathways but is chemically different: it’s a non-peptide small-molecule GLP-1, which means it can be absorbed orally rather than requiring an injection.

In clinical trials, patients taking this oral GLP-1 therapy lost around 11% of their body weight on average.

While this is not as powerful as the injectable GLP-1 medications, it is still very clinically meaningful weight loss and may suit many patients who prefer tablets or who require a different treatment approach.

Why this matters:
• More treatment options for patients
• A potential alternative for people who prefer not to inject medications
• Another tool clinicians can use as part of structured medical weight management

Importantly, medications are just one part of treatment. Sustainable results come from a comprehensive approach including:
• medical assessment
• nutrition support
• metabolic health optimisation
• behaviour and lifestyle change

The medication has just launched in the United States and is currently progressing through regulatory approval in Australia, with availability here likely around 2027.

Nausea is a common side effect experienced by many patients taking weight loss medications. While it can be uncomfortable, the good news is that nausea typically diminishes over time as your body adjusts to the medication. Here are some helpful tips to manage and reduce nausea while on weight loss medications:

• Stay hydrated - drink plenty of water throughout the day. Cold water is generally better tolerated. Small sips regularly are encouraged over large, quick gulps.

• Avoid greasy, highly processed foods - these foods can increase your risk of nausea and stomach upset. Lean proteins, vegetables and salads are better tolerated.

• Eat small meals regularly - eating small, protein and fibre rich meals is easier on the tummy than larger, heavier meals. Not eating at all can also cause nausea, so it's important to eat regularly, even when not feeling particularly hungry.

• Having a drink of water when you wake up can help settle any morning nausea.

• Eat more slowly - you will generally feel satisfied from an entree sized meal so eating slowly will help you to recognise when you are full which will help to prevent nausea related to overeating.

• Avoid lying down after a meal - this can cause nausea if the food refluxes back up the oesophagus. Staying upright can help the food to be digested easier.

As your body becomes accustomed to the weight loss medication, the initial side effects, including nausea, often decrease. This adaptation period varies from person to person but generally lasts a few days to a few weeks. During this time, following the tips above can make the transition smoother.
