Fortify Health Group Fortify Health Group is a specialist health, and disability organisation. Welcome to Fortify Health Group, a for-purpose health and disability organization.

We are on a mission to design, build, and deliver health and disability services that resolve complexity and empower individuals to achieve improved health and well-being. We support people from all walks of life, including children, adults, refugees, asylum seekers, international students, migrants, and their spouses, offering a range of specialist mental health programs and offer NDIS services that promote emotional and social wellbeing and reduce the risk of mental illness.

Our founders saw what happened when people with complex needs were asked to fit fragmented services, rigid pathways and models that did not see the whole person.

Fortify was built to do things differently, to bring clinical leadership, community knowledge and practical problem solving together around the individual.

That belief still drives us: every participant is unique, every journey is different and every model of care should be too.

The Mental Health Clinical Care Coordination program supports people experiencing severe and complex mental illness through coordinated, recovery-focused clinical care.

Fortify works with the person and their broader care network to strengthen coordination, clarify priorities and support continuity between clinical treatment and community life.

Referrals are currently available. Eligibility and pathway information can be discussed with our team.

Complexity does not scare us. It makes us curious. It gets us excited here at Fortify!

Fortify works with people and teams facing placement risk, fragmented supports, high-intensity clinical needs, behavioural complexity, repeated hospital presentations and barriers between systems.

We ask better questions, bring the right people together and keep working and pushing when the standard pathway is not enough.

We solve problems. We push past barriers. We challenge systems to work better around the person.

For people with complex needs, the capability of the person providing everyday support matters.

Around 80% of Fortify’s support workforce are Enrolled Nurses. This brings a strong clinical foundation into day-to-day care, supported by nursing leadership, individualised planning, competency and access to clinical guidance. There is very few areas in Fortify that our workforce does not have a clinical background. We are purposeful with this decision because it ensures from intake to providing care there is always clinical oversight to best support the person at the center.

It is one of the ways Fortify connects clinical oversight with what actually happens around the person each day.

We are often called when the usual pathway has stalled, the systems do not join up or a team needs stronger clinical direction. We bring people together, work through barriers and keep the individual not the system at the centre.

It represents the individual the person whose life, strengths, needs and goals should remain at the centre of every decision.

The two surrounding arcs represent informal supports, such as family and friends, and formal supports, such as clinicians and service providers. Neither circle is complete alone. Care works best when the whole support system finds balance around the person.
