📍Based in Caboolture and supporting our surrounding suburbs Our motto is nurturing and supporting participants so they can reach their full potential. Our focus and passion is to offer a trustworthy environment where participants can develop physically, socially, emotionally and access support to

develop skills to emotionally regulate and interact with others positively. Our camps are facilitated annually and offer qualified, skilled and accredited staff to enhance a learning environment for all. We also offer Support Coordination and a respite service from our home to give Carers a rest and to offer life skills, rest and fun for the participant.

School holiday fun is here! Heart to Heartbeat is excited to share our Week 1 and Week 2 Holiday Program, filled with fun, social connection, community access and new experiences.

🌟 WEEK 1
Monday 29 June – Urban Xtreme Hendra – $31
Tuesday 30 June – Dreamworld – $99 / Season Pass $149
Wednesday 1 July – Burpengary Creek Park & BBQ – Free
Thursday 2 July – Fishing at Sandgate – $10 bait
Please let us know if you do not have your own fishing rod.

Monday 6 July – Australia Zoo – $52 to $67
Tuesday 7 July – Movies: Minions and Monsters – $10
Wednesday 8 July – Strathpine Bowling & Pine Rivers Park – $19
Thursday 9 July – Elev8 Caboolture – $22

Our holiday program supports participants to build confidence, independence, friendships and community participation while enjoying safe and meaningful activities.

- NDIS Participants only
- School aged participants 6 years - 17 years
- Plan and Self Managed NDIS Plans
- Based in Caboolture and its surrounding suburbs
- Pick up and drop off within local locations (or along the route)
- 1:2 particpant to support worker ratio

School holiday recap! Joy, laughs, adventure, and a whole lot of friendship. 💃🏼💐🏊‍♂️
The past two weeks have been an absolute blast for our participants. We've had a supported holiday program, supported disability camp and a supported STR and our day to day supports.
We've been achieving some great goals, and working towards new ones in each day! 🌟 💓
