For young people leaving care, the transition to adulthood can feel overwhelming. Without the right support, it can be hard to build stability, develop skills, or know where to turn.

Open Minds Youth Bloom Transition Services support young people aged 15 to 21 across the Gold Coast and Logan regions to build everyday skills, make better decisions, and work toward independent living.
Through mentoring, practical support, and personalised guidance, our team walks alongside young people to help them set goals and move toward a more stable future. Support is available early, so young people do not have to wait until things become harder to manage.

Not sure where to go when things do not feel right?

For many young people, knowing where to start can be the hardest part. Whether it is stress, relationships, feeling overwhelmed, or something else entirely, you do not need to have it all figured out to reach out.

Through our headspace centres, young people aged 12 to 25 can access free, confidential support in a welcoming, non-judgmental space. Support is available across mental health, physical health, alcohol and other drugs, work and study.

Open Minds operates headspace centres at Indooroopilly, Redcliffe, and Strathpine.

Experiencing housing stress and need support?

For people in Redcliffe with mental health needs, finding and maintaining stable housing can feel out of reach. Open Minds, in partnership with Bric Housing, is here to help.

Our Specialist Homeless Services team works alongside you to access transitional housing and build the practical support needed to move toward more stable, independent living. From personal support planning and life skills to connecting with community and managing finances, we walk alongside you every step of the way.

To get started, submit an application through the Department of Housing. They will assess your situation and refer you to our team. For more information, call us on 07 3897 1829.

For people experiencing mental health challenges, securing and keeping suitable housing can be one of the hardest parts of everyday life. Open Minds Housing and Accommodation Support Initiative (HASI) is here to help.

HASI is a NSW statewide program that brings together support workers, clinicians, and the people who matter most to you, to build a personalised plan focused on your goals and your recovery.

Through this coordinated support, people develop everyday skills, build confidence, and move toward a more stable and independent future.

It can be hard to know where to start, especially when mental health challenges make everyday life feel difficult. Open Minds Community Living Supports (CLS) is here to help.

CLS is a community-based outreach program for people experiencing complex mental health needs in New South Wales. Our team works alongside you to build everyday skills, work toward your personal goals, and strengthen your connections with the people and places around you.

Using evidence-based approaches, we provide personalised support focused on what matters most to you and the life you want to build.

Available across Grafton, Lismore, and Tweed Heads.
