💙 What does a Care Partner actually do?

For many older Australians and their families, navigating aged care can feel confusing and overwhelming. A Care Partner helps make that journey clearer, more personal and less stressful.

Their role goes far beyond arranging services. A Care Partner can:

✔️ Listen to what matters to you
✔️ Help you understand your care plan, budget and contributions
✔️ Advocate when your circumstances or care needs change
✔️ Support you through reassessments and hospital discharge
✔️ Connect you with health professionals, assistive technology and home modifications
✔️ Help plan services so your funding is used effectively
✔️ Work with family members and others involved in your care
✔️ Identify supports you may not have known were available

Most importantly, a good Care Partner takes the time to understand your goals, preferences and changing needs, so your care continues to support the life you want to live.

At Care to Care, we believe care should never feel like a list of services. It should feel personal, coordinated and centred around you.

🤔 Confused about your Support at Home funding?

You’re not alone. Understanding your classification, quarterly budget and which services your funding can cover can feel overwhelming—but you don’t have to work it out by yourself.

Your Support at Home funding may include:

🔹 One of eight ongoing service classifications
🔹 A transitioned Home Care Package classification
🔹 Short-term funding for restorative care, assistive technology, home modifications or end-of-life care

For ongoing services, your funding is divided into quarterly budgets. Your approved services may fall within three main categories:

🩺 Clinical Supports
🤝 Independence Services
🏡 Everyday Living Services

Making the most of your budget starts with understanding:

✓ What services you have been approved to receive
✓ How much funding is available each quarter
✓ What contribution you may need to pay
✓ How your services support your needs and goals
✓ What your monthly statement is telling you
✓ Whether your current service schedule is making effective use of your available funding

At Care to Care , we can help you better understand your support plan, explore the services available within your approval and develop a care schedule that reflects what matters most to you.

💙 💙 Great News for Support at Home Participants!

From 1 October 2026, personal care services delivered under the Support at Home Program, including assistance with showering, dressing, grooming and continence support, will be fully funded under the Clinical Care category for eligible participants.

This means these important services will no longer require participant contributions once the changes take effect.

Personal care plays a vital role in supporting health, independence, dignity and overall wellbeing, and this funding change recognises the importance of these everyday supports.

Please note that current contribution arrangements will continue until 1 October 2026.
