HOME CARE PACKAGE CARE COORDINATION

Are you an older Australian wanting to stay independent in your own home and make the most of your Home Care Package?

At Sammi.K Support Services, we provide personal and responsive Care Coordination, working alongside Trilogy Care⁠ as your Home Care Package provider.

So, how does it work?

🤎 Trilogy Care is your Home Care Package provider and manages the package administration, funding and provider requirements.

🌿 Sammi.K is your Care Coordinator. We work directly with you to understand what you need, coordinate your services and supports, communicate with providers and help make sure your package is working for you.

We can help coordinate things such as personal care, cleaning, gardening, allied health, transport, equipment and other approved supports, depending on your individual needs and package.

The aim is simple — to help you get the care and support that matters to you, while maintaining your choice, independence and control.

Many people receive approval for a Support at Home Package but then feel overwhelmed by the next steps. With so many providers, services, and decisions to make, it's easy to wonder where to begin.

Working alongside Trilogy Care, I support older Australians and their families to:

Today I received some fantastic news for one of my aged care clients.

They have been approved for a fully funded hi-lift bed and hi-lift recliner chair through their My Aged Care supports—with no out-of-pocket cost for the equipment.

These types of supports can make such a difference, helping people stay safer, more comfortable, and more independent in their own homes.

This is exactly why I love what I do. Navigating the aged care system can feel overwhelming, but there are supports available that many families don’t even know exist.

Every person’s circumstances are different, and funding is always based on individual assessments and eligibility. But if you or someone you love is struggling at home, it’s worth finding out what assistance may be available.

NEW - AEI App 🧏‍♀️ 🏥 👮 🔥
For generations, Deaf and hard of hearing Australians have faced barriers in emergency situations - often forced to rely on others or navigate inaccessible systems. Since Triple Zero (000) was first introduced in 1961, equal access to emergency services has never existed - until now.

In an emergency, you can open the AEI App and press and hold the red circle for three seconds. You'll be instantly connected to an Auslan interpreter via video. They'll then contact 000 and relay your message for Police, Fire, or Ambulance.
