This specific middle-of-the-night waking is one of the most common complaints I hear from women in clinic.
Here is what is happening behind the scenes:
1. Progesterone & GABA: Progesterone naturally stimulates GABA receptors in the brain, our neurotransmitter for deep, calm sleep. As progesterone declines during perimenopause, sleep becomes lighter and more easily disrupted.
2. Blood Sugar Dips: If blood sugar drops too low during the night, your body releases a micro-surge of cortisol and adrenaline to wake up the liver and restore glucose. That cortisol surge is what leaves you suddenly wide awake and feeling alert at 2 AM.
A simple bedtime tweak: Try eating a small, high-protein snack 30 minutes before bed (such as a spoonful of almond butter, a small dollop of Greek yoghurt, or a handful of pumpkin seeds). This keeps blood sugar steady through the night and helps prevent that cortisol wake-up call.
