⏱️ The first 72 hours at home can expose every gap in a discharge plan.

🔍 During the first three days, the receiving team should know the person’s baseline, confirm medications and equipment, check that care instructions match the home environment, observe for changes and keep communication open with the participant and family.

⚠️ Common problems include missing consumables, unclear responsibilities, workers unfamiliar with equipment, inconsistent instructions and no agreed response when the person's condition changes.

🛡️ A strong transition plan anticipates these pressure points before discharge and includes early follow-up after support begins.

💡 The goal is not zero adjustment. The goal is a responsive team that notices issues early, communicates and acts.

🏥 Leaving hospital can be a relief and still feel overwhelming. For people with complex needs, the first days at home depend on dozens of details working together: trained workers, medication information, equipment, transport, clinical instructions, consumables, home access and clear escalation pathways.

📅 Throughout September, we are focusing on what makes hospital-to-home transitions safer and less stressful for participants, families and the professionals coordinating care.

Participants with high intensity needs require workers with demonstrated clinical capability. Providers have to be able to evidence that capability. So the pool of workers who can take those shifts is small — and the people in that pool get called first.

If you want to be in that pool, the pathway is clinical education. Not eventually. Now, while the sector is actively working out how it evidences competency.
