Support coordinator opportunity.

Gold Coast based, Coordinated Care and Support Services, is a registered provider, with a focus on quality support coordination and quality of life for out team and the clients that we service.

We are looking to expand our capacity and ensure quality, focussed coordination for our clients, through engaging an experienced local SC as a contractor.

We service in person South Brisbane to Ballina, with some remote clients and, though still a small team, our good reputation for quality, integrated support results in regular, good quality referrals.

We are quality rather than growth focussed, but that does bring some organic growth, so we are open to the right person joining our team.

Due to the experience in our organisation, we support a number of people with psychosocial, as well as physical disabilities.

Experience in mental health and neurodiversity, as well as collaborating as part of a multidisciplinary team is essential, as are great communication and organizational skills.

Allied health qualifications (Social worker etc.) and openness in having a mixed role as part of allied health team is not essential but is an option for people who like diverse roles.

If you can work autonomously day to day, whilst communicating well and integrating as a part of a team, have a really strong outcome for client focus and want to be a part of a progressive organisation, we would love to hear from you.

We pay 80% of all client billables.

May suit a current independent coordinator, with capacity for additional clients and who would like to be part of a team. May also be seen as a good opportunity for an independent SC to align with a registered organization for safeguarding against future sector changes.

To all of our clients and partners,

I hope that this message finds you well and that, as 2025 comes towards an end you have some positives to reflect on for the year that has passed and some things to look forward to over the Christmas period and into 2026.

To allow our team to recharge and come back next year ready to support you and your families, we will have a reduced activity period from Saturday 20th December 2025 until Monday 5th of January 2026.

During this time, general coordination and therapeutic service support will not be available.

Our central email [email protected] will be monitored periodically and support will be provided with urgent disability related matters.

If you need assistance to plan disability supports, or anything else, in between now and Friday 19th of December, please reach out to your coordinators as soon as possible.

Support Coordinator Seeking Independent Support Worker in Eagleby, QLD for mature female client.

Seeking experienced, skilled and genuine carer for NDIS client requiring flexible assistance including personal care (assistance to shower), help around home, some community access.

May include food prep, organization and tidying, adaptable to needs of the client.

Thursdays:

4 hour shift weekly

Saturdays:

Week 1 4pm - 8pm
Week 2 4pm - 7pm

Pay: $60 / hour weekdays, $80 / hour Saturdays

Applicant will be experienced, professional, positive and friendly. Good sense of humour ideal.

Maclean area, NSW.

Support coordinator seeking support worker to assist client for 3 hours / day, Sunday, Monday and Tuesday, typically 1pm - 4pm (with flexibility), who is resident in Maclean Hospital.

Client is a mature woman with dementia and requires 3 hours / day of support from someone kind, experienced in caring with someone with dementia, physically mobile, insured and safety checked.

The role involves engaging with the client within the hospital area (approved by hospital and NDIA), taking her for visits to the dining room for tea and biscuits, sitting on her with her balcony listening to music or engaging the client with activities such as showing pictures in books etc. Also includes having a walk within the hospital areas, including an in-hospital garden area.

Orientation and introduction provided by client's husband and existing support worker.

Ready to start ASAP.

This service is separate from Coordinated Care and Support Services and administered to the same principles of providing high quality tailored NDIS supports to meet a person's needs.

Support Coordinator Seeking an Independent Support Worker, in the Pimpama area, to assist an 18-year-old girl with community access supports on Fridays and Saturdays (4-6 hour shifts).

$50 / hour weekdays
$60 / hour Saturdays

Ideal candidate will have a good sense of fun, be young at heart, with good energy, patient and gentle natured, whilst able to advocate for the client if needed.

Experience of working with young people of a nervous disposition would likely be very helpful.

Seeking someone who is wanting to form a longer-term relationship, as the client takes time to connect with and build trust with new people.

Yellow card / blue card, full driving license and comprehensively insured car as well as professional insurance are essential.
