SPINAL PAIN: UNDERSTANDING YOUR TREATMENT OPTIONS

Why does pain sometimes return after an injection? And how do cortisone and PRP differ?

In our latest article, Dr Zane Sherif explores the role of CT-guided spinal injections, why accurate diagnosis matters, and how different treatments can support patients experiencing sciatica and nerve-related pain.

Cortisone can be valuable for reducing inflammation and relieving pain, while PRP offers a different approach that is being investigated for its potential role in tissue recovery.

Neither treatment is right for everyone. Understanding the cause of the pain is an important first step in determining the most appropriate approach.

An injection shouldn’t be the automatic answer every time pain returns. āœ‹ šŸ’‰ šŸ™…ā€ā™€ļø

Cortisone has an important role in reducing inflammation and helping some patients return to movement and rehabilitation but temporary pain relief doesn’t necessarily mean the underlying problem has resolved.

We believe treatments should start with understanding the cause of the pain, identifying the right target and considering what the treatment is intended to achieve.

For some patients, cortisone may be appropriate. For others, PRP or a different treatment pathway may be worth considering.

Our goal isn’t to just make pain disappear but to make informed decisions about the best path forward.

At MBR Health, we combine specialist radiology, advanced imaging and targeted treatment to help guide those decisions.

Treatment suitability and outcomes vary between patients. PRP is an evolving area of medicine and is not appropriate for every condition.

CT technology continues to evolve, offering new ways to look at the body.

At MBR Health, our advanced Philips Spectral CT technology provides additional information alongside conventional CT images, helping radiologists better understand certain findings.

In some cases, this may help clarify a finding and reduce the need for further imaging.
It’s not about replacing standard CT. It’s about having access to additional information when it may be useful.

At MBR Health, eligible standard diagnostic CT scans are bulk billed, with no CT gap fee.

Did you know your chest CT may also provide valuable information about your breasts?

Our Non-Compression Breast Spectral CT uses a specialised prone positioning protocol to produce detailed 3D images of the breast without mammographic compression.

And there’s something particularly interesting about Spectral CT too! Silicone has a distinctive spectral signature which means Spectral CT can provide material specific information that may assist in identifying silicone and assessing breast implants.

For some women, this can be an opportunistic approach, particularly when breast imaging can be incorporated into an existing CT examination being performed for another clinical reason.

It may be useful for selected women who:
• Have breast implants
• Cannot tolerate breast compression
• Are unable to undergo MRI
• Have dense breasts
• Are already undergoing CT imaging for another reason

It isn’t about replacing mammography, ultrasound or MRI, it’s about recognising that there may be additional opportunities to assess the breasts and implants with the imaging already being performed.

The scan shown is an example an MBR Health developed protocol, and the first specialised prone Spectral CT breast protocol of its kind in Australia.

Could nebulised PRP have a future in respiratory care?

This is an emerging area of regenerative medicine that we’re actively exploring at MBR Health.

PRP has been extensively investigated in areas such as musculoskeletal medicine and dermatology, but its potential use in respiratory medicine is a much newer field.

In our recently published 2025 case reports, Dr Zane Sherif and colleagues describe two patients who received nebulised autologous PRP for complex respiratory conditions.

The reports observed changes across measures including CT imaging, peak expiratory flow, symptoms and functional capacity.
These findings are interesting, but they are early observations, not proof of efficacy.

PRP is not a standardised product, and questions remain around platelet dose, preparation, aerosol delivery, lung distribution, safety and which patient populations may potentially benefit.

The next step is better research: well-designed prospective studies, clearly characterised PRP preparations, standardised delivery protocols and rigorous safety and clinical outcome monitoring.

At MBR Health, we believe emerging medicine should be approached with curiosity, scientific rigour and clinical restraint.

Our published case reports are one small contribution to a much bigger research question:
Could nebulised PRP have a future role in respiratory care?

Your PRP preparation starts well before the needle. 🩸

Because PRP uses your own blood and platelets, we want to create the best possible environment for your body to do what it’s designed to do, which is repair and recover.

In the weeks leading up to your procedure, focus on the basics that support recovery: eat well with plenty of whole, nutrient dense foods, keep added sugar and highly processed foods low, stay hydrated and prioritise good quality sleep.

It’s also a good time to minimise alcohol and avoid smoking, both of which can work against a healthy recovery environment.

For appropriate patients, we also recommend considering hyperbaric oxygen therapy as an adjunct to support tissue oxygenation and circulation, and starting beforehand may be beneficial.

And importantly, talk to us and/or your treating clinician about anti inflammatory medications before your procedure, as some can affect platelet function. (Never stop prescribed medication without medical advice.)

If you’re investing the time and effort into PRP, preparation is an important part of the process. The goal is to give your body the best possible environment to respond because your results don’t start on the day of the procedure, they start in the weeks leading up to it.

That pain chart you fill out before your scan has a good reason why so we ask you to take your time with it. 😊 āœļø

Where you feel pain, whether it travels, and how it behaves can provide important clinical context when your images are reviewed.
Dr Zane assesses the imaging and anatomical findings, then considers these alongside the patient’s symptoms and pain pattern.

Do the imaging findings correlate with what the patient is experiencing?

This is important because not every finding on an MRI is necessarily responsible for a patient’s symptoms. The aim is to look at the whole clinical picture, rather than viewing an image in isolation.

Your pain chart can also help inform the most appropriate imaging approach and, where relevant, the structures or areas that may need closer assessment.

So when we ask you to fill it out, please don’t rush it!!

The more accurately you can describe your pain, the more useful that information can be as part of your overall assessment.

Pain can be more complicated than just identifying where it hurts and pain felt around the hip, buttock, leg or lower back can have different potential sources, and the spine may sometimes be one part of the puzzle.

At MBR Health , our interventional radiologist, Dr Zane, considers the patients clinical history, their imaging and relevant anatomy, and their pain chart when determining whether an image-guided injection may be appropriate and what the appropriate target may be.

The aim is to understand the bigger picture and, where appropriate, use image guidance to precisely target the structure being assessed.

If you’re experiencing persistent pain or an injury, explore our Pain & Injury information on our MBR Health website via the link in our bio.

*Individual assessment is required to determine whether imaging or an image guided intervention is appropriate.*

A normal MRI doesn’t always mean there’s no reason for your pain.

And sometimes, an MRI finding isn’t actually the cause of your symptoms.

Disc bulges, degenerative changes and other findings can be seen in people who have no pain at all. Equally, some sources of pain may not be obvious on routine imaging.

That’s why good imaging starts with a good clinical question.

Your symptoms, clinical assessment and imaging findings all need to be considered together because the goal isn’t simply to find an abnormality.

It’s to understand whether that finding is relevant to you.

At MBR Health, our radiologists work closely with referrers to tailor imaging to the clinical question and help provide meaningful information for patient care.
