Complex Care & Support Services (CCSS)
Complex Care & Support Services is looking for an experienced and motivated Level 2 Support Coordinator to join our growing team.
This is initially a part-time position with the opportunity to grow into a full-time role as our Support Coordination caseload expands.
🏡 About the role
The position will primarily be work from home, offering flexibility around your workload. However, you will be required to attend face-to-face meetings with participants, families and other stakeholders when needed.
Our current Support Coordination participants are predominantly located throughout the Logan and surrounding areas.
We currently have a small but growing Support Coordination caseload, so we are looking for someone who is comfortable not only managing existing participants but also actively helping to grow our Support Coordination service and bring new participants on board.
🌸 We’re looking for someone who:
• Has experience in NDIS Level 2 Support Coordination – preferred
• Has a strong understanding of the NDIS and participant-centred practice
• Is confident communicating with participants, families, providers and allied health professionals
• Can work independently and manage their own caseload effectively
• Is organised, reliable and has strong documentation skills
• Is comfortable networking and developing referral relationships
• Is motivated to help grow their caseload and our Support Coordination service
• Is available to attend participant meetings in person when required
• Shares our commitment to providing genuine, responsive and individualised support
