What is Level 3 Support Coordination?

Specialist Support Coordination (Level 3) is designed for participants with more complex situations who require a higher level of support to reduce barriers, navigate multiple systems, manage risks, and build long-term capacity. This can include involvement with mental health services, justice systems, housing instability, crisis situations, safeguarding concerns, and complex support environments.

Our experienced team works collaboratively to help participants strengthen their supports, increase stability, and work toward their goals.

Our Social Work services within the NDIS space can include:

🔹 Future planning
🔹 Housing support and advocacy
🔹 Navigating complex systems
🔹 Domestic violence safety planning
🔹 Counselling and emotional support
🔹 Skill development
🔹 Crisis intervention
🔹 Assessments and reports
🔹 Evidence and recommendations to support funding requests for Behaviour Support

We also currently have capacity to provide:

Are you a passionate and experienced Support Coordinator looking for a role where you can truly make a difference? We’d love to hear from you.
At Support to Thrive, everything we do is guided by our core values - kindness, transparency, innovation, inclusion, and collaboration. We are deeply committed to providing high-level, participant-centred support and empowering people to live meaningful, self-directed lives.
We’re currently looking for a Support Coordinator based in Hervey Bay (or surrounding areas) to join our dynamic and supportive team. This is a permanent part-time role (25–30 hours per week), with the opportunity to grow into full-time.
In this role, you’ll:
- Support participants to understand and implement their NDIS plans
- Connect people with the right supports and services
- Build capacity and confidence for independent decision-making
- Work collaboratively with families, carers, and stakeholders
- Monitor supports and maintain high-quality records and reports
We’re looking for someone who:
- Has at least 12 months experience in Support Coordination
- Communicates with empathy and clarity
- Can work independently while being a strong team player
- Is genuinely passionate about empowering people with disability

Support to Thrive have been investing in some wonderful art pieces by our local artist Ebony Esparon-Binjuda. Ebony is not only and amazing artist, she is a wonderfully kind and compassionate human. If you are looking for some devine pieces of art, we highly recommend her. See her bio below and some pictures of her amazing work attached.
My name is Ebony Esparon-Binjuda, and I’m a proud Kaurareg and Waayni-Garrawar artist. I was born in Cairns on Gimuy land, raised on beautiful Butchulla Country, and I have strong family connections to Thursday Island in the Torres Strait and to Boodjamulla Country (Lawn Hill). These places, their stories, and their spirit shape who I am and the art I create. I’m currently in Year 11 at Fraser Coast Anglican College, and art has become my way of staying connected to my culture while growing into the person I want to be. When I paint, I feel close to my identity, my family, and the ancestors who guide me. My dot work, colours, and patterns express emotions and cultural stories that words can’t always hold. Every piece I create carries meaning; resilience, belonging, healing, and the spirit of Country woven through it. Creating art grounds me. It makes me feel proud, connected, and whole. And when my artwork finds a new home, I hope it brings warmth, meaning, and a story that can be felt as deeply as it is seen. My art is my voice, my connection, and my way of keeping culture strong and alive.

We are neuroaffirming practitioners who work collaboratively with participants, families, and key stakeholders to:

Minimise and reduce restrictive practices
Support meaningful, strengths-based outcomes

✨ Why Safeguards Are in Place for Restrictive Practices and PBSPs ✨

We often hear that supports and families feel frustrated by the “bureaucratic processes” that come with restrictive practices (RP) and Positive Behaviour Support Plans (PBSPs). So, we wanted to explain why they’re in place — and why they matter.

Restrictive practices — like physical holds, environmental restrictions, mechanical restraints or medication for behaviour management — carry risks. They can affect someone’s independence, safety, and quality of life. That’s why safeguards exist:

✅ To protect human rights – Everyone deserves dignity, choice, and respect. Restrictions are only ever used when absolutely necessary, and in the least intrusive way.

✅ To keep people safe – Every use of a restriction must be monitored, documented, and reviewed so it’s not misused or overused.

✅ To focus on positive support – PBSPs are designed to reduce the need for restrictions over time by building skills and strategies.

✅ To ensure accountability – Clear approval and review processes mean the person’s best interests are always the priority.

At their heart, safeguards are about protection, not barriers. They aren’t here to make life harder — they’re here to make sure every decision respects the person, keeps them safe, and puts them at the centre of their support. 💙

Life threw Nyree some of its toughest challenges — surviving not one, but two strokes while raising a young family and juggling work — yet through sheer determination, hard work, and countless hours of rehabilitation, she not only returned to full-time employment but has gone even further by building and running her own business. We’re so proud to share that Nyree was recently nominated for the Fraser Coast Ability Awards, and it has been truly inspiring to watch her kick goal after goal this year. Nyree, your resilience and drive light the way for so many others, and we couldn’t be happier for all that you’ve achieved!
