Thats a wrap from IFSO Toronto 2026!

A timely meeting to help everyone align the future direction of obesity management.
There was alot of new clinical trial data on medical therapy being presented, highlighting both the pros and cons of GLP-1 therapies.

The messaging from this conference was clear - the way of the future is going to be integrated obesity care through multi-modality therapy in treating a chronic disease process. The available treatment options are expanding and we as clinicans need to embrace change and effectively guide our patients.

The yearly IFSO meeting is a great time to catch up with close friends and colleagues from across the world! Already looking forward to next year's IFSO asian pacific chapter in Singapore and IFSO world congress in Paris!

Let’s reflect on some of the key messages from this year’s world conference.

Here are 9 key messages highlighting results from some of the major clinical trials and ever changing landscape in obesity management.

in 2026, we have quite alot of options when it comes to weight loss management.

A question that needs answered is "whether gastric balloons can still cut it in 2026??"

Hear from Dr Cheng about his CANDID OPINION about gastric balloons and whether it is still worthwhile for you.

When it comes to weight loss outcomes, we are seeing GLP-1 medications match the results
achievable by Gastric balloons. Both have question marks over weight loss sustainability unless GLP-1 medications are used long term.

Gastric balloons remains a NICHE weight loss treatment tool in patients who have had sub-optimal outcomes with GLP-1, yet hesitant about undergo bariatric surgery. Is the balloon for everyone? Not quite but does remain an option in bridging the therapy gap between medication and surgery.

For decades, BMI has been one of the main ways we define and measure obesity.

But we are starting to understand that our focus is flawed.

BMI doesn’t tell us:
• How much of that weight is fat vs muscle
• Where the fat is distributed
• How much intra-abdominal fat is present
• Whether we are preserving muscle during weight loss
• What our metabolic health looks like
• How obesity is affecting our function and quality of life

Two people can have the same BMI but very different body composition, health risks and treatment needs.

Recent evidence and IFSO guidelines are moving towards driving management according to clinical obesity which places more emphasis on obesity related metabolic disease (like diabetes, heart disease, high blood pressure, sleep apnoea) rather than a single BMI number.

As obesity management evolves, we also need to be much more obsessed with body composition which accounts for muscle, fat and fluid percentages.

We should move away from just asking:

“What is your BMI?”

And start asking:

“What is your body composition?”

BMI can be a starting point.
It shouldn’t be the whole story.

It’s time to look beyond the number.

Time absolutely flies!

Last year this time, we were in Chile Santiago for the IFSO 2025 Conference.

This year, Dr Cheng is heading to IFSO Toronto in September to join up with fellow metabolic colleagues from around the world.

The International Federation for Surgery of Obesity and Metabolic Disorders (IFSO) conference is arguably the biggest bariatric conferences held in different global cities each year. It brings together leading specialists from across the world where we gather to learn, share knowledge and discuss the current/ future management of obesity.

What hernia surgery is now is vastly different to even a decade ago.
We now have so many different techniques on how to tackle abdominal wall hernias which often translates to better outcomes for our patients.
Advancements in robotic surgery has really revolutionised the hernia surgery field which allows us to tackle most hernias through a minimally invasive approach.

The Shire is finally getting our very own surgical robot!
