Moorditj Koort Aboriginal Corporation (MKAC) is a leading Aboriginal-led, not-for-profit organisation committed to improving the health and wellbeing of Aboriginal and Torres Strait Islander peoples across Western Australia. We offer a number of services to the community, including the Moorditj Koort Care Coordination program that helps Aboriginal and Torres Strait Islanders manage their health and wellbeing, in particular for those who have a chronic disease. Our aim is to be a professional, independent, all-inclusive organisation that offers culturally secure health and wellbeing services to our local Aboriginal community. We will do this by developing and strengthening the relationship between local health services, agencies and the community, and to improve the planning, evaluation and delivery of health services.

Culture Care Connect Program (CCCP) โค๏ธ๐Ÿ’›๐Ÿ–ค

Supporting Aboriginal people across the Perth metro region with culturally safe, confidential, and accessible care when it's needed most.

CCCP provides:
โ–ช๏ธ Early intervention support
โ–ช๏ธ Aftercare services
โ–ช๏ธ Connections to health, wellbeing, and community supports
โ–ช๏ธ A 6-month recovery-focused program to strengthen wellbeing and reduce the risk of harm

Our Aftercare Officers walk alongside clients to build self-care strategies, strengthen support networks, and reconnect with culture, family, and community.

This month, Moorditj Koort joins the call to raise awareness about bowel cancer โ€” Australiaโ€™s second deadliest cancer.

Every week, bowel cancer claims the lives of 101 Australians, yet it is one of the most treatable cancers when detected early. Early screening can save lives.

We encourage our community to:
โœ…Know the signs and symptoms
โœ…Complete regular bowel screening tests
โœ…Yarn with your doctor or health worker if you have concerns
โœ…Support family and loved ones to get checked
