At Gratitude Home Care, Care Management is focused on practical coordination of services that genuinely improves quality of life.

Our care team has supported Anna through a package upgrade and transition from Home Care Package to Support at Home program services.

The Care team has also supported her to access to services including gardening, cleaning, osteopathy, and a falls alarm, with supports gradually expanding over time.
Anna is now happier, safer, and more comfortable in her home and body, with a cleaner living environment, reduced loneliness, and renewed independence.
