Seeking specialist disability accommodation (SDA) – Northern suburbs.

We are currently supporting a participant who has SDA funding approved and we are looking for a suitable home in the Melbourne northern suburbs, preferably the following :
Craigieburn
Roxburgh park
Glenroy
Coolaroo
Fawkner
Broadmeadows
Campbellfield

Addition requirements/information:

Building type: House, not shared.
Funding: maximum approved amount of $83,304
Participant has his own Support Worker, not looking for one.
Ground level home to support safe wheelchair access and mobility.
Needs to move as soon as possible.

Male Participant aged 16 years - requires support worker to assist with daily and community activities.

Must have cert 3 qualification, WWCC, criminal check, NDIS worker screening and first aid and public liability insurance.

A Male participant aged in his early 60s , requires a support worker to assist with daily and community.

Must have cert 3 qualification, WWCC, criminal check, NDIS worker screening and first aid and public liability insurance .

Wednesday from 10am to 3pm
Saturday from 3pm-8pm
Sunday from 10am to 8pm

A female participant aged in her early 40s , requires a support worker to assist with community support.

Support worker will be required to take participant to community events such as going to shopping centre and take her to her work place in Dockland .
Must have cert 3 qualification, WWCC, criminal check, NDIS worker screening and first aid.

Wednesday to Friday
Start 12pm onwards
(6 hours daily)
Sat and Sun
5 hours
