To help maintain and grow this standard, we are seeking an experienced Care Coordinator particularly with experience in the National Disability Insurance Scheme, Aged Care, Community Nursing and/or Home Care Sector to provide high quality services and oversee our day-to-day operational activities.
As an ideal candidate, you will have a passion for providing community-based care underpinned by robust quality standards. An element of the role will include attending shifts with clients to ensure you keep current on in-home care processes and procedures. This will enable you to perform quality checks on the standard of care we provide to our community’s most vulnerable.
Key factors to t his role includes monitoring and managing the intake process, which includes rostering and scheduling of client services. Responding to and appropriately triaging all incoming requests for service. Agility to successfully support the broader team that develop, maintain, and build positive relationships with our internal carers, participants/patients, their families, and external parties. Providing administrative support to the Care Coordinators as required. The role is responsible for creating and refining policies and procedures in line with regulatory requirements, as well as overseeing the progress, and review of service agreements and billing and supervision of staff associated with these streams. The successful applicant will possess highly developed people skills; they will be motivated, compassionate, and able to make this role their own by utilising their demonstrated work experience in the disability, nursing, or human services sector.
Reporting to our Operations Manager who is committed to the development of their people, you will lead and be part of the client services team where you will be required to deliver outcomes against a designated caseload. You will also be responsible for supporting a multi-disciplinary team of professionals who are looking after NDIS Participants and other clients. Additionally, you will display a proven ability to develop and maintain an environment of trust, diversity, and inclusion within your team.
