Capillary refill comes back under 3 seconds.

Reassuring.

But as you look closer the skin has a redness and discolouration to it that isn’t tracking like a bruise.

Something isn’t right.

This is a presentation that catches even experienced injectors off guard.

A normal CRT does exclude a vascular occlusion.

In a venous VO, blood can still be entering the tissue but struggling to drain.

The result is a skin change that looks inflammatory or mottled rather than the classic blanched white of an arterial occlusion.

CRT is one tool. It was never designed to be the whole assessment.

A cadaver study by De Lima Faria et al., found that even experienced plastic surgeons correctly identified their intended injection plane in only ~11% of cases.

Perceived anatomical accuracy does not always reflect reality.

Cannula use does not eliminate risk.

Incorrect plane assumptions, tissue movement, pressure, and anatomical variation can still lead to delayed or atypical complications.

Reducing risk requires more than technique.

It requires deeper anatomical understanding, plane awareness, and careful clinical judgement.

Not all vascular occlusions present the way you would expect. ❌

After hyaluronic acid filler treatments in areas like the chin, most practitioners check skin colour, capillary refill, and pain levels post-treatment at a surface level,

but they often overlook the oral cavity. 👇🏻

Intra-oral signs like blanching or discolouration, can be early indicators of a vascular occlusion.

These signs are subtle, can be very small/ localised, meaning they are easily missed, and often progress before external symptoms appear.

Failing to check areas such as the gingiva, inner lips, tongue, and palate (pre & post treatment), could delay critical intervention.
