Iron deficiency, low vitamin B12, an underactive thyroid, vitamin D deficiency and undiagnosed diabetes can all present as fatigue long before anything else becomes obvious. Iron deficiency is particularly common in women of reproductive age and often goes unaddressed for years. The symptoms—tiredness, difficulty concentrating and breathlessness on stairs—are easy to attribute to a busy life. If fatigue has been the norm for more than a few weeks, bloodwork is a reasonable early step. It may identify something treatable or rule out a whole category of causes—both useful outcomes.

Your circadian system determines when you feel alert and when you feel sleepy, taking most of its cues from light. When those signals are inconsistent, your body clock drifts out of step. This has a name: social jetlag. Waking at 6.30am on weekdays and 10am on weekends has much the same physiological effect as crossing time zones twice a week, and is associated with poorer daytime function regardless of how many hours you sleep. Monday morning fatigue is often your body clock still running on Saturday’s schedule. Morning light is the strongest cue your body clock has. Getting outside within an hour of waking, even briefly, tends to do more for daytime energy than an extra half hour in bed.

Sleep Health Week wrapped up on Friday, and it prompted a question we hear a lot: why am I still tired when I'm sleeping enough?

The campaign does good work on sleep. What it can't fully cover is the tiredness that isn't really about sleep at all.

Fragmented sleep never reaches the deeper stages where recovery happens, and around one in ten Australians has undiagnosed sleep apnoea without knowing. But plenty of fatigue has nothing to do with the bedroom. Iron deficiency, thyroid issues and vitamin D deficiency all present as tiredness long before anything else shows up. A body clock that drifts across the weekend produces much the same effect as jet lag. And mental fatigue doesn't respond to the same rest as physical fatigue.

This month's newsletter is about fatigue rather than sleep. Five common causes, what's worth checking for each, and the signs that tiredness warrants a proper conversation with a clinician.
