This role is ideal for someone with a background in case management, social work, intake coordination, community services, or health care navigation who is passionate about supporting patients and families at critical transition points.
As the Hospital Liaison Manager, you will play a key role in supporting hospital teams and patients to plan safe, timely discharges. You’ll work closely with hospital clinicians and Prestige’s care coordination teams to identify appropriate supports, coordinate services, and ensure continuity of care beyond discharge.
This is a relationship‑focused, client‑centred role, prioritising advocacy, system navigation, and coordination.
This position is a 12-month maternity leave contract, available for a commencement at short notice.
