What matters most to you at the end of life?

In health and aged care settings, conversations about preferences for end-of-life care often happen in the middle of a crisis.

They don’t have to happen that way.

The ‘What Matters Most’ Discussion Starter booklet and cards support older people using aged care services to have important conversations about their future care preferences - before a crisis happens. Designed to be used alongside health or aged care staff, the resources help older people, families and carers explore what matters most to them if they become seriously ill or near the end of life.
