📢 Important NDIS changes commenced yesterday — 27 August 2026

New legislation affecting the NDIS has now come into effect, with several changes particularly relevant to participants, Support Coordinators, Recovery Coaches, Plan Managers and providers.

One of the most important changes relates to unscheduled plan reassessments.

From today:

🔹 Only the participant, plan nominee or child representative can request an unscheduled plan reassessment.

🔹 Support Coordinators and providers can help participants prepare their request and explain their changed circumstances, but can no longer submit the reassessment request on their behalf.

🔹 A reassessment should generally relate to a significant and ongoing change in functional capacity, living arrangements, education/employment or informal supports.

🔹 Short-term or urgent circumstances may instead be addressed through a plan variation, where appropriate.

🔹 The NDIA will have up to 90 days to decide whether to undertake a reassessment.

The legislation also introduces stronger integrity, accountability and record-keeping requirements, including stronger information-gathering powers and consequences for false, misleading or deceptive conduct.

For providers, this is another reminder of how important strong documentation, communication and governance are across every level of NDIS service delivery.

At Your Story Community Supports, we’ll continue staying across these reforms and working collaboratively with participants, families and stakeholders as the changes are progressively implemented.

This open day is an opportunity for NDIS participants, families, guardians, support coordinators and allied health professionals to:

• Discuss participant compatibility and transition planning
• Ask questions about our person-centred support approach

Meet Milan – Our Clinical Coordinator 👋

We’re pleased to introduce Milan, Clinical Coordinator at Your Story Community Supports (YSCS).

Milan is a Registered Nurse with experience across nursing, disability support and leadership roles, with much of her career dedicated to supporting people with complex health and support needs.

She is passionate about person-centred care, strong clinical practice and empowering teams to deliver safe, consistent and meaningful support. Milan brings a collaborative and practical approach to problem-solving and values continuously learning from participants, families and the people she works alongside.

“I’m passionate about person-centred care, building strong clinical practices, and finding practical ways to support both participants and the teams around them. I’m looking forward to getting to know our participants and teams and contributing to safe, high-quality and meaningful support.”

As Clinical Coordinator, Milan will work closely with our participants, families and teams to strengthen clinical governance, staff capability, health-related supports and quality and safeguarding practices across YSCS.

We’re excited to have Milan joining the YSCS team and look forward to the knowledge, experience and leadership she brings to our services. 💙

Welcome Donna – Our New Service Delivery Manager

We are delighted to welcome Donna to Your Story Community Supports (YSCS) as our new Service Delivery Manager.

Donna brings more than 20 years of experience across the community and human services sector, with a strong focus on disability services and supporting people to live meaningful, empowered and more independent lives.

Throughout her career, Donna has remained passionate about what matters most the people at the heart of our sector. She is committed to creating environments where people with disability are respected, heard and valued, and where they are supported to pursue their goals and live life on their own terms.

In her new role, Donna will work closely with our team to strengthen service delivery, support sustainable growth, expand our reach and continue building YSCS's presence across the disability sector.

Donna's approach combines strong operational leadership with purpose-driven service delivery, ensuring that as we continue to grow, quality, integrity, innovation and positive participant outcomes remain at the centre of everything we do.

We're excited about this next chapter and the experience, leadership and passion Donna brings to the YSCS team.
