If you’re an NDIS provider, documentation is not just admin, it is part of protecting your participants, your team and your business.
Clear records help show what support was provided, why it was needed, how risks were managed and whether the right processes were followed.
These 5 areas should be documented clearly:
1. Participant Support Needs
Document each participant’s goals, daily support needs, communication preferences, risks, routines and any changes over time. This helps staff provide safer, more consistent and person-centred support.
2. Participant Service Agreements
Service agreements should clearly outline the supports being provided, responsibilities, pricing, cancellation rules, consent and how services will be delivered. This helps avoid confusion and supports transparency.
3. Incident Management Records
If an incident occurs, records should show what happened, who was involved, how the team responded, what follow-up was completed and what actions were taken to reduce future risk.
4. Staff Training & Compliance Checks
Providers should keep records of staff induction, qualifications, training, supervision and ongoing compliance checks. This helps show that workers are prepared, competent and supported in their roles.
5. Quality & Safeguarding Policies
Policies around complaints, incidents, risk management, participant rights, worker responsibilities and safeguarding should be documented, reviewed and followed in practice.
It is not about ticking boxes, it is about making sure participants receive support that is safe, respectful and properly managed.
