Some patients have seen everyone, and still don't have an answer.

The file is thick. The specialists were thorough. And yet the thing that's actually wrong β€” the exposure, the metal burden, the post-viral tail no one followed up β€” is sitting in a blind spot, because it was never the question anyone thought to ask.

It isn't the tired of one bad night. It's the tired that's still there after the weekend, after the early night, after the holiday you were sure would reset you. You've probably stopped mentioning it, because "I'm just tired" gets a nod and not much else.

The most popular calm-your-cortisol herb of the moment just picked up a caution.

The TGA has updated its advice on ashwagandha (Withania), after a small number of reports of possible liver injury. To be fair to it: the risk looks very rare, and the Australian cases with enough detail resolved once the product was stopped. Rare isn't the same as trivial, though β€” when it does happen it can be serious, and ashwagandha is best avoided if your liver is already under strain. This isn't a scare β€” it's a reminder.

"Natural" is not the same as "no effect." Anything strong enough to help is strong enough to be worth checking against your history, your medications and your liver.

That's the quiet argument for having someone actually review what's in your cupboard.

The virus passed months ago, but the fatigue didn't. Standing up is a decision. Thinking clearly is a project. And the tests, mostly, say you're fine.

A label tells you what to call your problem. A mechanism tells you what to do about it.

Standard care is built for speed β€” ten minutes, a script, the next patient. It works for a great deal. But it can leave one kind of patient behind: the one whose tests keep coming back "normal" while their life quietly doesn't.

Integrative and functional medicine is not an alternative to that care. It runs in parallel to it. The difference is time and depth β€” a full history, the right testing, and a plan built from what the evidence in front of us actually says.
