* May arise from any tissue in the hand
- skin, subcutaneous fat
- muscle, nerves, vessels
- tendon, bone and cartilage

*Most are benign, asymptomatic and may not require surgical intervention.
However if diagnosis is in doubt, or malignancy is suspected, early referral is essential.

*Ganglions, cysts, giant cell tumours of the tendon sheath, and swellings associated with arthropathy comprise the majority of lesions.

Orthopedic tests for the SIJ

How useful are they?

I found myself researching this after a discussion with a colleague yesterday.

Most clinicians learn a few pain provocation tests for the SIJ at uni and early in their career.

But they don't realise that these tests are not enough to rule in or diagnose SIJ related pain.

These tests just are not specific enough to the sacroiliac joint.

Attention should be focused on key features of the subjective exam such as pain nature, aggregating activities and medical history.

After this information is analysed, then the orthopedic tests may the useful.

When assessing for vertebral alignment- look no further that the 4 radiographic parallel lines.

1. Anterior cortical or vertebral line: anterior portion of vertebral bodies and connection point of the anterior longitudinal ligament (ALL).

2. Posterior cortical or vertebral line: posterior portion of vertebral bodies and connection point of the posterior longitudinal ligament (PLL).

3. Spinolaminar line: posterior margin of the spinal canal and line of ligamenta flava.

4. Posterior spinous line: tips of the spinous processes or line of the supraspinous ligament.
