Thousands of vessels in the face, only a small number named. It’s almost impossible to not puncture one on the way in or out.

The question was never whether you’ll hit a vessel. Its whether you hit of the important ones in a way that can cause a complication.

Why is the cosmetic consult free? You pay for most other consults in medicine.

Have free consults inherently trained injectors to sell? Maybe the part that matters more gets rushed, and the part that pays gets amplified.

We know money creates bias, and bias can push a patient toward more treatment, but the right treatment?

I ran my numbers last month and advertising was the biggest line on the sheet. More than stock. More than rent.

Four or five years ago the money I was putting into promoted posts, had genuinely impressive reach. Today that same amount does close to nothing, and matching the reach I had then costs several multiples of what it did.

That is Meta or Google knowing exactly what a patient is worth to us now. So they price it at whatever still leaves us marginally ahead, and keep the rest.

New injectors are walking into a version of this industry where paid acquisition is a serious cost centre from day one.

Two questions that can be asked of every new patient.

1. Does the way you look concern you?
2. Do you spend a lot of time thinking about your looks?

They take about a minute and they’re the difference between noticing something before you treat and finding out afterwards.

From our position as injectors, it isn’t really about a diagnosis. Most of the patients she’s cautious about have never been diagnosed with anything. It’s the person back for the fourth time this year about one crease, and the honest response to that isn’t a refusal, it’s asking what’s actually going on.
There’s no clean line between the patient who’s committed to their plan and the patient who can’t stop. It’s a spectrum, and you’re standing somewhere on it every time you pick up a syringe.
The small number of patients where the answer is no will never make or break your week. The one you should have said no to can absolutely make or break your year.

The free consult is a broken model, likely from an origin that is commercially driven. But it’s also the best hour you’ll ever get with the patient.

While most people are skimming over it, and heading straight to the treatment, use it to your advantage and see it as an opportunity to build a relationship, and a lifetime patient 🫶

A study came out saying patients who got a facial assessment were two and a half times more likely to come back.

Sounds great. But here’s what I do before I get excited about any number like that: I ask who paid for it.

Because this one was funded, and funded studies have a habit of finding flattering results. That doesn’t make it wrong. It just means we read it differently.

And honestly? Even setting the study aside, a proper facial assessment being the thing that keeps patients coming back isn’t surprising to us at all. It matches what I see. The study is just a bonus.
