Triniti Home Care, a registered NDIS provider operating across the Central Coast, Lake Macquarie and Macarthur regions, is seeking an experienced Support Coordinator to join our growing team in a full-time, permanent position.
In this role youll support participants to understand and implement their NDIS plans, coordinate a range of supports, and build strong, trusted relationships with participants, families and providers. Youll be backed by a dedicated operations and clinical governance team and the systems to do the job well.
Essential
Experience in NDIS support coordination
Working knowledge of the NDIS framework, pricing arrangements and CoS practice
Strong written documentation and time management
Current NSW WWCC and NDIS Worker Screening Check (or ability to obtain)
Valid drivers licence and reliable vehicle
Desirable
Existing relationships across Central Coast and Hunter networks
Experience supporting participants with complex needs
