Sprained your ankle - do you need a scan?
The first question should always follow a detailed assessment & ask - "will imaging change how we manage the injury?"
Many uncomplicated ankle sprains can be assessed and managed without immediate imaging. When a scan is needed, the correct choice depends on the injury suspected.
π©» X-ray is usually first when a fracture is possible.
γ°οΈ Ultrasound can be useful for superficial ligaments and dynamic tendon problemsβbut it cannot adequately assess every bone, cartilage or joint injury.
π§² MRI provides the most comprehensive assessment of ligaments, tendons, cartilage, bone marrow and the syndesmosis, without ionising radiation.
π CT provides detailed information about fractures, bone architecture and joint alignment.
π₯And what if you heard a crack?
It sounds alarming, but a crack or pop does not reliably distinguish a fracture from a ligament or tendon injury. The location of tenderness, ability to bear weight and findings from a clinical assessment are more useful.
The best scan is not necessarily the easiest one to book. It is the scan that answers the clinical question and has the potential to change treatment.
