Deciding when a participant needs an OT assessment isn't always straightforward, especially when the picture is mixed or the information you've got feels out of date.

A few situations worth flagging for yourself: functional capacity that seems to be shifting without a clear reason, a plan review or AT request coming up where you'll need solid evidence behind it, or supports that don't seem to be landing the way they should.

Cognitive, behavioural or psychosocial factors affecting day-to-day function are easy to underestimate too, particularly when they're not the primary diagnosis.

The clearer you can be in a referral about what's changed, the impact on daily activities, and the participant's own goals, the more useful the resulting assessment will be, both for planning and for any funding conversation that follows.

If you're sitting on a case where something isn't quite adding up, that uncertainty is usually the clearest sign an assessment would help.

It's not always obvious when it's time to bring in an occupational therapist with your elderly loved one, and often the signs build up gradually rather than arriving all at once.

Someone taking longer in the shower, avoiding the stairs, or quietly giving up on tasks they used to manage themselves are all worth paying attention to, as are one or two falls that get brushed off as "just a stumble."

The same goes for carers.

If assistance is creeping from occasional to constant, or manual handling is starting to feel risky, that's a signal too, not a failure on anyone's part.

An OT assessment at this point isn't about waiting for a crisis, it's about understanding what's changed, what's driving it, and what practical options exist, whether that's equipment, a home modification, or a different approach to a daily task.

Earlier input generally means more options, not fewer.

If something feels different lately for someone you're supporting or caring for, it's worth trusting that instinct.

We are more than an occupational therapy provider.

We are a team of people who genuinely care about the work we do and the difference it makes. For the participant who can finally move safely through their own home. For the older person who gets to stay in the place they love. For the coordinator or case manager who gets a response when they reach out and a report they can actually work with.

That is the mission. Not just to deliver a service, but to show up for every person in our world in a way that actually matters.

22 years of doing this has not made us complacent. It has made us more certain about what good looks like, and more committed to delivering it.

When you're referring a complex case, the real risk usually isn't finding a provider, it's ending up with a report that doesn't hold up once it reaches the NDIS.

A few markers of a report that will actually do its job: every recommendation ties clearly back to functional need, the participant's goals are reflected in their own words, and it answers the obvious follow-up questions before a planner has to ask them.

Before you refer, it's worth asking a provider how they handle scope changes mid-assessment and how quickly they turn around a report once the on-site work is done.

Timeframes and communication matter just as much as clinical expertise once you're the one managing the file.

Finding the right occupational therapist for someone you love can feel overwhelming, especially when you don't know what a good fit actually looks like from the outside.

A few things worth asking before you commit: how quickly can they start, will you see the same therapist consistently, and can they explain their recommendations in plain language rather than clinical shorthand.

It's also worth asking what happens if needs change partway through, because a good provider should be able to adjust the plan rather than starting again from scratch.

Trust your gut too.

If a provider is vague about timeframes or cost, or the first conversation feels rushed, that's worth paying attention to.

The right fit is someone who makes you feel heard, not just booked in.

We all know the saying “Prevention is better than cure,” and when it comes to home safety for older people, it’s so true!

“It’s easy to wait until something happens before we act, but is this the right thing to do and does it lead to a positive outcome?”

Falls accounted for approximately 32% of all injury hospitalisations among older Australians.

Slipping, tripping, or stumbling on the same level was the most common cause, resulting in 73,730 hospitalizations (287 per 100,000 population).

Falls involving stairs or steps accounted for 7% of hospitalisations​.
