This Breast Cancer Awareness Month, I want to share a message of hope grounded in the progress we have made.

I’m Dr Lucia Saliba, and as a breast surgeon, I want people to know that a breast cancer diagnosis today comes with more treatment options and better prospects than it did decades ago.

In Australia, five-year relative survival for breast cancer has improved from 75% in the late 1980s to 93% for people diagnosed between 2017 and 2021. These are population figures, rather than a prediction for any individual, but they offer a very encouraging picture.

Surgical care has progressed too. For many women with early breast cancer, breast-conserving surgery followed by radiotherapy offers equivalent survival to mastectomy. Where appropriate, sentinel lymph node biopsy can also reduce the risk of arm complications compared with more extensive lymph node surgery.

Oncology treatment is increasingly tailored to the characteristics of each cancer. Chemotherapy, hormone treatments and targeted therapies give us different ways to reduce recurrence risk and improve outcomes when clinically appropriate.

And emotional wellbeing matters. Hope, connection and psychological support can help people cope and improve quality of life. But you do not have to feel positive every day. Fear, sadness and uncertainty are normal, and they do not mean you are letting yourself down.

Every diagnosis is different, and treatment has risks and side effects that deserve an honest discussion.

This October, let’s recognise the progress, keep supporting research, and remind anyone facing breast cancer that they deserve both expert care and compassionate support.

How well do you know what is normal for your breasts?

Breast awareness doesn't require a complicated self-examination routine.

Cancer Australia recommends becoming familiar with the normal look and feel of your breasts so that you are more likely to notice a new or unusual change.

Changes can include a new lump or swelling, a change in breast size or shape, skin redness or dimpling, ni**le changes or discharge, swelling around the armpit or collarbone, or unusual breast or ni**le pain that doesn't go away.

These symptoms do not automatically mean breast cancer. There are many other possible causes.

But if something is new or unusual for you, talk to your doctor.

And even if you attend regular mammograms, continue to be breast aware between screenings.

Breast Cancer Awareness Month is a useful reminder, but breast awareness matters throughout the year.

Breast cancer screening is one part of a much bigger story.

Since BreastScreen Australia began in 1991, the age-standardised breast cancer mortality rate among Australian women aged 50–74 has fallen from 74 deaths per 100,000 women to 37 per 100,000 in 2023.

That reduction should not be attributed to screening alone. The Australian Institute of Health and Welfare notes that it reflects, in part, earlier detection through BreastScreen Australia alongside advances in breast cancer management and treatment.

Screening can also detect cancers while they are small. In 2023, 6,410 invasive breast cancers were detected through BreastScreen Australia among participants aged 50–74, and 57.8% were 15 mm or smaller.

In NSW, women aged 50–74 are invited to have a free breast screen every two years, while women over 40 are eligible for free screening.

Importantly, screening is designed for people without breast symptoms. If you notice a new breast or ni**le change, speak with your doctor rather than waiting for your next routine mammogram.

Receiving a message asking you to return after a screening mammogram can understandably cause concern.

But being recalled for further assessment is not the same as being diagnosed with breast cancer.

BreastScreen Australia data from 2023 show that 10.6% of participants aged 50–74 attending their first screening round were recalled for further assessment, compared with 3.8% attending subsequent screening rounds.

Further assessment may involve additional mammogram images, examination, ultrasound and, when required, a biopsy.

Importantly, most people recalled did not have an invasive breast cancer detected.

If you have been recalled, follow the instructions provided by your screening service and ask your healthcare team any questions you have about the next steps.
