Direct Care Resources is looking for a Manager - Care Planning and Practice to join our team.

This role will see you supporting care planning and practice across our Disability and Child Safety services, working closely with support workers, health professionals and operational leaders.

You'll help ensure care is planned, delivered and reviewed effectively, while providing guidance on complex support needs and supporting teams to deliver consistent, high-quality care.

We're looking for someone with experience in care or case management who can communicate clearly, work collaboratively and contribute to positive outcomes for the people we support.

Direct Care Resources is looking for a Senior Case Manager (Senior MCPP) to join our team.

This role will see you leading and mentoring a team of Care Managers while providing guidance on complex disability and child safety cases. You'll work closely with families, stakeholders and external agencies to support positive outcomes for children and young people, while helping ensure our services continue to meet the highest standards of quality and compliance.

You'll also play an important role in workforce development, supporting recruitment, training and capability building across the team.

We're looking for someone who can provide strong leadership while building positive relationships across teams, families and support networks.

You’ll often hear these terms used in SIL — but they’re not always explained clearly.

At Direct Care Resources, 7/7 and 4/3 refer to how support workers are rostered, with an option for longer blocks of time with the same support worker rather than frequent daily changeovers.
