From hospital to his very own home — we are so proud to share this beautiful feedback from our participant Darren. 💙

"I first met my Sublime Care staff in November 2025 while I was in the process of being discharged from hospital. They helped me access the community and do the activities that I wanted to do. Such as going to the beach, community centres, and the movies.

Once I left hospital I initially moved into community housing where I continued to receive Sublime Care community support which enabled me to remain more independent and continue to attend my appointments, access the community, and build on my life and living skills.

In June 2026, after many months of planning, Sublime Care offered me a place to live. I was nervous to live on my own at first however I now have a calm supportive environment to live in where I can learn lasting skills and enjoy my retirement years!" 💙

From assessment to independence — this is what our OT team does! 💙

We are so proud to share this wonderful outcome for our client Trish, with her full consent and blessing. 🌟

Trish was referred to our Senior OT Aimee through My Aged Care (CHSP) in South West Sydney. After an initial assessment and a joint site visit with builder Stuart from BaptistCare Home Modifications & Maintenance Service, Aimee completed a full OT home modification report and the results speak for themselves!

A brand new ramp was installed at Trish's home, and here is what she had to say:
💬 "I'm delighted with the ramp and it makes it easy for me to do my own washing instead of depending on Linda. I can also reach my car more easily. Baptist Care did an excellent job and I recommend them."

But that is not all. Through the Get2Go equipment program, Aimee was also able to source:
✅ Personal care aids
✅ Adaptive cutlery and utensils
✅ A kettle tipper

All to help Trish live more independently every single day. 💙

This is what person-centred aged care looks like. We are so proud of Aimee and the whole team involved in making this happen for Trish! 🙌

Since 2019, we've built a dedicated support team, walked alongside families across Newcastle, and supported over 100 NDIS and Aged Care participants across New South Wales. Behind every number is a person, a family, and a life we're grateful to be part of. 👨‍👩‍👧‍👦💗
