Based in Sale (with regular travel to Warragul), you'll work closely with participants, families and providers to build capacity, coordinate supports, and empower people to live their best lives, with greater choice and control.

Would you like to build confidence, independence and practical life skills in a supportive group environment? CCV is excited to launch Expressions of Interest for our upcoming Capacity Building Group Programs, designed to help people develop everyday skills, increase independence, and work towards their NDIS goals.

Our small, supportive groups may include:

🏑 Independent Living Skills & Everyday Communication
Learn skills such as following daily routines, managing household tasks, making appointments, asking for help, and communicating confidently with family, support staff and community members.

πŸ’™ Emotional Regulation & Wellbeing
Develop strategies for recognising emotions, managing anxiety and frustration, building self-confidence, practising mindfulness, and creating healthy wellbeing routines.

πŸͺ₯ Everyday Self-Care & Hygiene
Build independence with personal hygiene, grooming, healthy sleep habits, choosing appropriate clothing, and managing personal belongings.

πŸ“š Functional Literacy & Communication
Increase confidence reading everyday information, completing forms, understanding emails and text messages, using calendars, and building basic computer and communication skills.

πŸ₯— Healthy Living, Cooking & Nutrition
Learn meal planning, grocery shopping, budgeting, food safety, preparing healthy meals, and making nutritious food choices.

🚌 Independent Travel & Community Access
Build confidence using public transport, planning journeys, reading timetables, practising road safety, accessing local services, and participating in your community.

🀝 Social Connections & Participation
Develop conversation skills, make new friends, understand social cues and boundaries, work as part of a group, and participate confidently in community activities.

We understand that Daily Living Disability Services in Traralgon has recently closed without notice, which has understandably disrupted supports for many people in the community.

Moments like this can feel overwhelming, especially when supports change so suddenly. It’s important that people feel supported, informed, and able to make choices about what comes next.

If you or someone you know has been affected, CCV is here to help. We can provide Support Coordination and Psychosocial Recovery Coaching, working alongside you to navigate your options, reconnect supports, and plan next steps in a way that suits you.

We’re excited to officially introduce Community Connect Victoria (CCV) πŸŽ‰

CCV is a new part of the Disability Connect Victoria family. We’ve been created as a dedicated new business area, with a clear focus on coordination, recovery, and helping people navigate complex systems with confidence.

This step came from listening. Over time, we saw the need for a more specialised approach to Support Coordination, Recovery Coaching, and Aged Care Coordination. Creating CCV allows us to focus fully on this work, while staying closely connected to DCV and the values that shaped us.

You’ll still see the same experienced team, the same person-centred approach, and the same commitment to doing things right. What’s new is our focus and capacity to support more people across Victoria.

As CCV grows, we’re planning to expand into additional services, including Behaviour Support, group activities, and more... watch this space.

We’re excited about our new journey and can't wait to show off what CCV can offer!
