If you've spent any time in the disability sector, you've heard 'person-centred care' a lot.
It's in every provider's brochure. It's in the NDIS framework. And it's used so often it's started to lose meaning.
So what does it actually mean?
It means the plan starts with you
Not with what's easiest to roster. Not with what other participants typically receive. With you your goals, your preferences, what a good day actually looks like for you.
It means you can change your mind
Your needs shift. Life changes. Person-centred care adjusts with you. It's not a document you sign once and forget.
It means we listen every time, not just at the start
Regular check-ins. Open feedback. A team that actually acts on what they hear.
That's the standard we hold ourselves to. If it ever falls short, we want to know.
