What is Mate-Match? 💛

It’s our friendship program for people with disability on the Central Coast, a chance to meet others who share your interests and build meaningful connections at your own pace.

We get to know you, look for people you could click with, and help organise time together doing things you enjoy. Think coffee catch-ups, games, craft or local outings, with a community mentor supporting your small group. ☕🎨🌿

Whether you’re ready to meet someone new or feel a little nervous about getting started, we’re here to help.

You don’t need to have all the answers. What matters is making time, listening without judgement and following up.

ILO and SIL can sound similar, but they offer different kinds of support.

🌿 ILO is a flexible arrangement designed around how, where and with whom a person wants to live.

🏠 SIL provides structured and consistent support for people with higher support needs, to live safely, confidently and as independently as possible.

If SIL isn’t suitable or funded, ILO may be another option worth exploring. Every person is different, and will depend on their individual circumstances.

Real progress happens when support workers, support coordinators, families, therapists, behaviour supports, allied health teams, and providers come together with one shared focus, the participant and their goals.

Every stakeholder brings a different piece of the puzzle.

When communication is open, strategies are aligned, and everyone works together, participants are given the best possible opportunity to build independence, confidence, skills, and meaningful outcomes.

Collaboration means:
- Consistent supports
- Better communication
- Stronger goal progression
- Reduced stress for participants and families
- A team approach that empowers people to succeed

ILO (Individualised Living Options) is an NDIS support that helps people build independence and create a living arrangement that suits their lifestyle, goals, and support needs.

This can include learning everyday life skills, building confidence in the community, exploring future housing options, and receiving tailored supports while living independently, with family, or with others.

At Platform To, we support participants through practical, real-life skill building such as cooking, routines, budgeting, transport training, community access, and working towards personal goals, helping people feel more confident, capable, and independent at their own pace.
