- Diagnosis: Large PDA- 5mm with Small ASD 2 Severe coarctation of aorta. A nightmare case.
- Procedure: Aortogram showed a large tubular PDA filling both branches of the pulmonary arteries. Initially, the aortic stump was not clearly visualized. Therefore, the wire was re-crossed into the ascending aorta, and repeat aortography was performed, which demonstrated severe preductal coarctation of the aorta (CoA).
Initially, balloon coarctoplasty was performed using an 8/2 Tyshak-II balloon. Following this, PDA device closure was attempted with an 8/6 MFO device; however, the device slipped out and could not be stabilized.
Subsequently, a larger 8/7 muscular device was selected and positioned successfully. Before releasing the device, an attempt was made to compress the aortic retention disc using an 8/2 Tyshak-II balloon across the coarctation segment. During balloon inflation, significant balloon wedging was noted due to contact with the device, and displacement of the aortic disc inward was not possible. Pressure measurements taken across the coarctation segment revealed a significant residual gradient of 35 mmHg. Therefore, the muscular device was recaptured and removed.
Thereafter, a 10/8 ADO-I device was selected and positioned successfully. Prior to release, repeat balloon compression was performed across the coarctation segment, which created adequate space around the device. Repeat pressure measurements taken above and below the device showed a residual gradient of 10 mmHg. The device was then successfully released under balloon protection.
