☘️A high vitamin B12 level in a blood test does not always mean your body's cells have enough usable B12. This can happen for several reasons:

🍁1. Inactive B12 is being measured. Most standard blood tests measure total B12, which includes both active and inactive forms. A high result may reflect inactive B12 that your cells cannot use.

🍁2. Functional B12 deficiency. Even with a high blood B12 level, the vitamin may not be entering cells properly or may not be functioning correctly. In this situation, the body can still behave as if it is B12 deficient.

🍁3. Recent supplementation or injections. Taking B12 supplements or receiving B12 injections can make blood levels appear very high for weeks or months, even if tissues are still recovering.

🍁4. Underlying health conditions. Liver disease, kidney disease, certain blood disorders, inflammation, or some cancers can cause elevated blood B12 without indicating better B12 status.

🍁5. Binding protein abnormalities. High levels of B12-binding proteins (such as haptocorrin) can increase total B12 in the blood while active B12 remains normal or low.

⭐If a person has symptoms of B12 deficiency despite a high blood B12 result, doctors often check:

👉Methylmalonic acid (MMA) – a better marker of cellular B12 deficiency.

👉Homocysteine – often elevated in B12 deficiency.

👉Holotranscobalamin (Active B12) – measures the form of B12 available to cells.

👉Folate, iron, and a complete blood count (CBC), depending on the clinical picture.

☘️This situation is particularly recognized in some people with chronic inflammation, gut disorders, mitochondrial dysfunction, or certain metabolic conditions, where total B12 may not accurately reflect how much B12 the cells can actually use.
