This case involves a 53‑year‑old woman who presented with a symptomatic epiretinal membrane (ERM) in the right eye, accompanied by pronounced metamorphopsia that significantly affected her daily visual function. At presentation, her best‑corrected visual acuity (BCVA) was 20/60, consistent with tractional distortion of the foveal contour.
Comprehensive preoperative imaging was obtained, including color fundus photography, wide‑field retinal imaging, and macular OCT, all of which demonstrated a dense epiretinal membrane with inner retinal wrinkling, loss of foveal contour, and tractional changes extending beyond the central macula. These preoperative images are included for comparison.
The patient underwent pars plana vitrectomy with meticulous ERM and internal limiting membrane peeling. The procedure was uneventful, and postoperative recovery progressed as expected.
By postoperative visit number 15, her BCVA improved to 20/40, with a marked reduction in metamorphopsia. Postoperative retinal photographs and OCT scans — also included for comparison — demonstrate:
* Restoration of the foveal contour
* Resolution of tractional striae
* Normalization of the inner retinal layers
* Improved macular reflectivity and architecture
