The Highland Primary Care Network (HPCN) has been around since 2007 providing health care services in Airdrie, Carstairs, Crossfield, Didsbury and surrounding areas. Documenting your preferences for your health care and personal matters helps to prepare your loved ones for the future. 🤍 This is called Advance Care Planning. You can make sure the people you trust know what to do if there is ever a time that you are unable to make or communicate decisions yourself.

There are lots of free and low-cost community resources to help you with this process, including your health care team. A PCN social worker can help answer your questions and provide you with guidance around completing these documents.

Q: What’s one simple change people can make to improve their overall health?

A: Increase exercise/physical activity. Physical activity not only helps maintain a healthy weight, reducing the risk of diabetes and other cardiovascular diseases, but also improves mental health and mobility as we age.
