The Upper Canada Family Health Team is comprised of a group of health care professionals working collaboratively to provide comprehensive and accessible health care. The goals of the Upper Canada Family Health Team include preventing illness and promoting the health of patients. We aim to reduce the number of patients in Brockville who do not have a health care provider, reduce ER visits for minor illness, and reduce hospital admissions for chronic illnesses.

Please note that Upper Canada Family Health Team sites and the After-Hours Clinic will be closed on Monday, September 7, 2026, in recognition of the Labour Day Holiday.
We will reopen on Tuesday, September 8, 2026, and resume regular office hours.

📢 Mallorytown Unattached Patient Clinic – Closing October 30, 2026

With the provincial government's ongoing investment into primary care, including multiple providers actively accepting new patients in the community of Mallorytown, the Unattached Patient Clinic is winding down and will officially close its doors on Oct. 30 2026.

We are proud of the work that Dr. Redekopp and his team have accomplished at the Unattached Clinic over the past few years. We leave knowing that residents will be better served from long term comprehensive primary care moving forward.
