Every day, I ask patients what new indoor air environments they visited in the last week. Do you know what the answer almost always is?

“I haven’t been anywhere. “

So I ask questions like:
• When did you get groceries?
• What activities are your children engaged in?
• Did you go to church?

Inevitably, the answer to further inquiry reveals multiple visits to multiple indoor air environments. No one is intentionally lying, it’s just far too much to remember; especially when you have a CIRS-brain.

The solution? Make a log of all the indoor spaces you visit. Do regular VCS testing. This way you can confirm exposure after visiting (should your VCS worsen).

Generally, a failure to tolerate binders can be managed. You’re not doing anything wrong, you just haven’t yet considered the variables that trigger you.

Binders cannot outpace ongoing exposure.

If you’re still spending significant time in a water-damaged building or, if there’s a hidden source in your home or workplace you haven’t identified, you’re in a race where toxins are entering faster than binders can eliminate them.

This is the merry-go-round problem. You clear some. More comes in. Net balance: zero (at best).

Starting the Shoemaker Protocol is a big commitment. Choosing the wrong CIRS practitioner can mean wasted time, money, and prolonged illness. Choosing the right one can change your life.

CIRS is a clinical diagnosis supported by laboratory testing. Without the right diagnostic process, you risk being treated for the wrong condition—or missing key steps in your recovery.

What to look for in an answer:
• Uses symptom clusters (8 of 13 clusters suggests CIRS).
• Includes Visual Contrast Sensitivity (VCS) testing.
• Orders CIRS-specific laboratory tests, such as TGF-β1, MSH, C4a, MMP-9, VEGF, and HLA haplotypes.
• Does not rely on urine mycotoxin testing.

If you’ve been on a binder for six to eight weeks and aren’t seeing meaningful improvement, the environment needs to be reassessed before you start adjusting the binder.

ERMI or HERTSMI-2 testing of your current living space is the right first step. Air filtration helps, but it doesn’t replace source removal.

A safe environment is the foundation that makes everything else possible.

You don’t need to me tell you what to pack for your vacation. But I will add some specific items for the CIRS crowd.

These include:
• A portable air purifier. If you are flying, don’t trust it to your checked bag. Make sure it is small enough to carry on with you.
• A small hygrometer to check room humidity,
• Co2 monitor to check for fresh air (higher co2 levels = stale indoor air)
• Binders

I always recommend patients increase their binder frequency one to two days before travel. Make sure you continue taking binders througout your travels and again for another week after you return home. if you’re noticing a symptom flare, do a VCS to confirm whether or not it is caused by an exposure.

Whether you are considering a hotel or a rental, you need to see how much fresh air can enter the premises. With CIRS, we’re generally concerned about concentrated indoor microbial contaminants.

The solution to pollution is dilution.

If you can keep all the windows and patio doors open, you will significantly dilute indoor air contaminants. This makes for a much more CIRS-friendly indoor environment.

If you have CIRS, your innate immune system is in overdrive. Simultaneously, your mitochondria are in hibernation. This combination means you’re a prime candidate to have a symptom flare should you be exposed to biotoxins.

Once we add travel to the mix, you’ve now got the increased odds of stacking multiple exposures together in a very short period of time.

Consider all the indoor air environments you might encounter within the first day of travel:
• Departure airport
• Connection airport
• Arrival airport
• Hotel/Air BnB
• Subway/train station

Any of the above have the potential for biotoxins. And this is your exposure risk on the first day of travel. Add jet lag, eating off-menu and/or in restaurants, and a stressful day of missed connections… Your HPA axis is now in fight mode. Your mitochondria are on ice. If not done properly, you are are in an uphill battle from the moment you leave home.

With that in mind, let’s figure out how you can better travel with CIRS.

In this video, I walk through at a high level what belongings are typically considered safe to keep, which are case-by-case, and which are generally too contaminated to reliably clean.

This is an important, and often the most painful part, of remediation for many people.
