Injection depth matters… and your outcomes depend on understanding why.
When we talk about treating different areas of the face, it’s not just about where you inject. It’s also about which layer you are working in and understanding the connective tissue arrangements that connect the different facial compartments.
Many areas require different injection depths depending on the treatment goal, the anatomy, and, most importantly, the patient in front of you.
Some general principles I follow:
• Structural support is created deep — injected on bone/supraperiosteal, where the filler will live in the deep fat pad.
• Contour, refinement, and blending are created more superficially — injected subdermally, where the filler will live in the superficial fat pad.
• Certain areas require both — depending on the indication and treatment approach.
The forehead, piriform, gonial angle, and areas requiring structural cheek support are typically treated deep.
The lips, submalar region, preauricular area, and certain contouring aspects of the face are treated more superficially.
The chin and tear trough can require both depths, depending on the location and the approach.
Understanding layered facial anatomy — and knowing which depth serves the outcome you are intending to achieve — is paramount for creating safe, balanced, and natural-looking results.
Unnatural-looking results are not simply the result of “overfilling.” Often, even the smallest amount of product placed in the wrong layer can result in the strangest-looking appearance.
