Wild yam cream vs natural progesterone. What’s the difference?
Wild yam contains a plant compound - diosgenin - that’s taken into the lab and chemically modified to become identical to human progesterone. Indeed wild yam is the starting material of prescription bioidentical progesterone creams or pills.
However the human body can’t convert diosgenin into progesterone. It requires lab synthesis.
A wild yam cream does not contain progesterone nor can it claim the same benefits as bioidentical progesterone.
It’s good for moisturizing the skin though.

Inspired this week by a patient who called me “her ChatGPT” 😂 I asked chatGTP for the #1 perimenopause question women ask AI. And this was it… “Do these symptoms sound like perimenopause or is something wrong with me?”

It wasn’t about hot flashes or weight gain or hormone therapy - it’s women looking for reassurance that they’re not crazy. That there’s a reason for the exhaustion, rage, anxiety, overwhelm, heavy periods, sleepless nights, or the overall feeling of NFLM (not feeling like myself)

Perimenopause is a nuanced diagnosis taking into consideration age, symptoms, cycle length, and labwork to rule out other potential causes of your symptoms.

The most common symptoms associated with perimenopause (based on research-standard clinical questionnaires):
✨changes in menstrual cycle - more/less frequent; more/less flow; more/less difficult
✨vasomotor symptoms - overheating, changes in body temp, night sweats, heart palpitations
✨sleep problems-problems falling or staying asleep and sometimes both
✨mood changes - feeling more anxious, inner restlessness, feeling down, depressed, irritable, hypersensitive to sounds, smells
✨physical and mental exhaustion - decrease in performance, forgetful, problems multi tasking, trouble focusing
✨reduced libido
✨bladder/urination problems - increased frequency, stress incontinence, more frequent UTIs
✨vaginal dryness
✨vague joint and/or muscle pain

These are the most common symptoms that bring women into clinic. And the good news … they’re treatable! Diet, exercise, mindset and lifestyle matter. Correcting nutrient deficiencies and musculoskeletal issues matter. Hormone therapy can be a piece of the puzzle. Perimenopause is natural but that’s doesn’t mean it’s easy or that you need to suffer through it. Help is here if you need it🫶

Do you fast before your cholesterol tests? Both fasting and not fasting have pros and cons.

💫Eating before labwork can increase triglycerides, which are a storage form of energy you just consumed. Normally, triglyceride levels peak 3-4 hours after eating and clear from the bloodstream after 12 hours of fasting. Persistently high levels can signal metabolic issues like insulin resistance and pre-diabetes.

💫Elevated triglycerides can artificially lower your LDL cholesterol readings. In non-fasting tests, we rely on other markers like non-HDL and ApoB levels.

💫That being said, non-fasting tests have their own benefits. They can reflect real life since we're not fasting most of the day. Different triglyceride ranges are applied to fasted and non-fasted tests.

💫Non-fasting tests are also more convenient, especially for those prone to hypoglycemia.

For the best option for YOU, follow the recommendations of your clinician.

Heart palpitations are among the ‘vasomotor symptoms’ of peri/post menopause. However don’t automatically dismiss them.

📌any abnormal heart racing, fluttering or irregular heartbeat at any age warrants follow up with your medical team
📌palpitations accompanied by shortness of breath, feeling faint, chest pain or poor exercise tolerance will be investigated. Also, in women with strong personal or family history of heart disease
📌after other causes have been ruled out, palpitations associated with vasomotor symptoms in peri/post menopause are unlikely to be an immediate indication of heart issues however current data suggests women with vasomotor symptoms (hot flashes, night sweats, overheating) in general are at higher risk of cardiovascular disease over time and will need to be regularly screened and advised on prevention strategies.

Time and energy aren’t unlimited, so when I say yes to getting better at this or doing more of that, something else has to give.

In my house? That something is often housekeeping—cosy and comfy is the theme, not magazine clean. And ‘there appears to have been a struggle’ isn’t far off most days 😂

What will you be doing less of this year so you can say yes to something else?
