When it comes to bladder urgency, frequency, and nighttime waking in men, the symptoms don’t always tell us why they’re happening.

Is it:
✅ Idiopathic overactive bladder?
✅ Obstruction from an enlarged prostate (BPH)?
✅ Bladder muscle overactivity?
✅ Prostate or pelvic floor muscle tension?
✅ Lifestyle or behavioral factors?
✅ Or a combination of several things?

This is why proper assessment matters.

Rather than treating symptoms alone, it’s important to dig deeper and understand what’s driving them. Medical testing, a thorough history, and an assessment with a pelvic floor physiotherapist can help determine whether the muscles, bladder, prostate, nervous system, or bladder outlet are contributing to the problem.

The goal isn’t just symptom management—it’s finding the cause so you can build the right treatment plan and get better results.

Your bladder symptoms are giving clues. The key is making sure you’re asking the right questions.

5 symptoms I wouldn’t ignore as a man 👇

If you’re noticing changes in how you p*e, don’t just assume it’s part of getting older.

Here are 5 symptoms that deserve attention:

1️⃣ A change in your urine flow
Your stream isn’t as strong as it used to be.

2️⃣ Intermittent p*eing
Your stream starts and stops instead of flowing smoothly.

3️⃣ Incomplete emptying
You finish p*eing but still feel like there’s more left behind.

4️⃣ Hesitation
You stand there waiting for your urine stream to start.

5️⃣ Straining or pushing to p*e
You feel like you need to force the urine out.

These are all symptoms of voiding dysfunction and may be early signs of prostate-related changes.

And with how long it can take to see a specialist in Canada, it’s a good idea to start the conversation early.

Talk to your doctor.
Get some baseline testing.
Ask about a referral to a urologist.

Even if the symptoms are mild, establishing care early means you already have a plan in place if things progress.

Because when these symptoms are left too long, men can begin to develop storage symptoms such as:
• Urgency
• Frequency
• Post-void dribble
• Nighttime waking

And don’t forget—pelvic floor physiotherapy can be an important part of your treatment team, helping to support bladder function and quality of life alongside your medical care.

For those of you gentlemen who have undergone a prostatectomy, I’m really curious about your experience after the catheter was removed.

Did you experience:
• Immediate and fairly significant bladder leakage?
Or…
• More mild dribbling and occasional leakage?

In my practice, I commonly see men who are quite surprised by how much leakage they initially experience after surgery.

But I also imagine there are men whose leakage was much more mild from the start.

One of the reasons I’m asking is because I think it’s important for men to hear a variety of real experiences so they know they’re not alone in whatever recovery path they’re on.

This is something I see all the time in men.

This past week alone, I evaluated a few men with bladder symptoms, and one thing kept showing up over and over again…

When urination is working normally, you never stop to think about how it actually works.

But the moment symptoms start showing up — urgency, frequency, hesitation, nighttime waking — suddenly it’s:

“What the heck is happening down there?”

And here’s the interesting part:
Many men don’t even realize they have muscles that are deeply involved in the urination process.

So when I ask someone:
“Can you activate your pelvic floor?”
“Can you hold in gas?”
“Can you show me how you’d stop the flow of urine?”

…it’s often like there’s no software program installed yet for how to do it.

And that is incredibly common.

Not abnormal.
Not weird.
And absolutely retrainable.

As pelvic floor physiotherapists, part of our job is helping you:
✔️ Connect to these muscles
✔️ Learn how they work
✔️ Improve coordination and control
✔️ Use them more effectively to support bladder function

So if you’ve noticed changes in your bladder symptoms, this is your sign to go see a pelvic floor physio.

We have made so many advances in medicine for treatment of bladder and prostate symptoms.
Lots of medication options and surgical procedures.
But conservative management???
Why is there so little talk of it?
There is evidence, certainly not enough.
And pelvic floor physio… friends we gotta find a way to break out of the Kegel shackles.
We do so much more for our clients.
I wonder what it would look like for men if they had full collaboration between urology and pelvic floor physio?

5 signs your pelvic floor may be dysfunctional…
In men.
Yes men can pelvic floor dysfunction because they also have pelvic floor muscles.
If pelvic floor muscles were overlooked in women… they are definitely overlooked in men.
