Wolf Creek PCN

Wolf Creek PCN Welcome to the Wolf Creek PCN! We are part of your Patient Medical Home! PCNs develop solutions to meet the needs of the local community and their health issues.

Primary Care Networks (PCNs) bring local physicians and other health care professionals together to provide comprehensive patient care to Albertans. PCNs are comprised of groups of family physicians working with other health care professionals such as nurse practitioners, nurses, dietitians, pharmacists, social workers, exercise specialist, behavioral health consultants, and mental health professi

onals. PCNs are created through an agreement between physicians and Alberta Health Services. Please note that PCNs are not a governing body to physicians; the College of Physicians & Surgeons of Alberta regulates the practice of medicine in Alberta. There are 40 PCNs operating throughout Alberta with more than 3,800 family physicians, and more than 1,400 other health practitioners. At any given time, there are hundreds of PCN family doctors across Alberta accepting new patients. Research shows that patients who regularly visit a family doctor are healthier and live longer. The Wolf Creek PCN supports the Central Alberta communities of Bentley, Delburne, Innisfail, Lacombe, Ponoka, Rimbey, and Sylvan Lake. Going outside is a simple way to support your overall health. For both children and adults, spending time enjoying the outdoors is an easy way to boost mood, focus, and relaxation.

Connecting with nature doesn’t have to be complicated; even small moments can make a big difference. Go for a stroll around your neighbourhood, spend time in your garden, or simply take some deep breaths and listen to the birds around you. 🐦🌻

Looking to build healthier habits outdoors? Your primary care team can help you set realistic goals for physical activity, manage health conditions that may affect your ability to get active and connect you with programs and resources in your community.

May is Mental Health Awareness Month, a timely reminder that supporting mental well-being is essential in both our personal and professional lives. It’s an opportunity to reduce stigma, encourage open conversations and prioritize self-care while also checking in on colleagues, friends and loved ones.

By fostering understanding and compassion, we can help create environments where everyone feels supported and empowered to seek help when they need it. 💚

If you or someone you know needs support, consider talking to your healthcare provider about available mental health resources and services.

Today, on World Family Doctor Day, we are celebrating the incredible impact family physicians have on the health and well-being of our communities. 🩺 Thank you, Dr. Sheik for encouraging healthier lifestyles and helping patients build habits that support their physical and mental well-being at every stage of life.

Q: What’s one simple change people can make to improve their overall health?

A: Increase exercise/physical activity. Physical activity not only helps maintain a healthy weight, reducing the risk of diabetes and other cardiovascular diseases, but also improves mental health and mobility as we age.

This National Nursing Week (May 11–17, 2026), we proudly celebrate the incredible nurses who care for patients across Alberta.

From Primary Care Networks to community clinics, nurses play a vital role in supporting patients at every stage of their health journey, providing compassionate care, building trusted relationships and helping people navigate the health system.

This year’s theme, The Power of Nurses to Transform Health, recognizes the profound impact nurses have in shaping better outcomes for individuals and communities.

To all the nurses in our PCN and in clinics across our network: thank you for the knowledge, dedication and care you bring every day. Your work truly makes a difference.

Wherever life happens, safety matters. 🛡️
This week is Safety and Health Week, a time to focus on preventing injury and illness at work, at home and in our communities.
Simple steps can go a long way in keeping you and those around you safe:

✅Keep your spaces clear, well-lit and free of tripping hazards to help prevent falls
✅Practice everyday habits like handwashing and staying home when sick to reduce the spread of illness
✅Stay up to date with routine check-ups, screenings and vaccinations

Healthier communities start with safer environments. If you have questions about what you can do to help prevent injury or illness, talk to your primary care provider or health team.
