Hi everyone! My name is Jamie Sweetland and I am a Nurse Practitioner, located in the beautiful Annapolis Valley in Nova Scotia. I have worked in the community of Middleton and the surrounding area for the past 10 years and I love the people here. Specifically, over the past 2 years, I have worked mainly with the veteran population and have completed many VAC Medical Questionnaires, Disability Tax Credit applications, driver's medicals, insurance paperwork, plus many more types of medical documentation. Currently, a patient must see their family doctor/NP for paperwork, however many patients I meet do not have a family doctor and visit the emergency department or walk-in clinics to have their forms completed. Sometimes the wait times to see your own primary care provider are too long or your P*P just doesn't have the time. That's where I can help. The Nova Scotia government does not reimburse residents for completion of medical documentation, so unfortunately patients have to pay out of pocket for these services no matter where they go. My goal is to make it easier, more accessible, and faster to have your paperwork completed with me. Right now, clients can book online and receive a call from me within the week. I will review what paperwork you need completed and what documentation you will need to bring with you to your appointment to aid me in completion of your forms, if applicable. We will discuss pricing and procedure - you will meet with my nurse first for an initial assessment and then myself for the final signatures. Veteran forms can be billed directly to VAC and I accept all forms of payment for any other forms. Unfortunately I cannot accept patients as their primary care provider at this time.
