When the time comes to manage a respiratory failure patient, the differential diagnosis for the root cause will play a key role in the ability to adequately treat…
We are good assessing vital signs, auscultation for lung sounds as well as history gathering. We also have to ensure the understanding of medications and patient prescriptions, as that can help give the details in the clinical picture when we work to determine underlying issues and patient exacerbations.

CPAP works best when “the primary problem is oxygenation failure with recruitable alveoli—not primary ventilatory failure.”

That’s why “acute cardiogenic pulmonary edema remains CPAP’s most consistent win.”

But success in the field comes down to ex*****on:

• “Start low and titrate begin around 5 cm H₂O… then increase toward 10”
• “Seal check in three spots: bridge of nose, cheeks, and under the chin”
• “Reassess quickly: at 2–3 minutes… 5 minutes… and 10 minutes”

And remember, “a large fraction of apparent CPAP ‘failures’ are actually delivery failures—leaks, poor mask fit, or constant interruptions.”

Also worth repeating:
“If COPD (or CO₂ retention risk) is likely, avoid ‘blast oxygen until 100%.’”

Bottom line: “CPAP should not become a ritual.” Use it early, reassess often, and know when to move on.
