OutCare continues to support the hospices of the Champlain Health Region. Grants from the OutCare Fund will be awarded twice a year to area hospices.

The OutCare Board of Directors will transition to the OutCare Fund Advisory Committee and will continue to review and approve the bi-annual hospice grant applications.

OutCare is committed to supporting the area hospices.

We will continue our twice annual grants. The grants will come from the OutCare Fund administered by the Ottawa Community Foundation. You can donate to the OutCare Fund at the link: https://www.ocf-fco.ca/funds/outcare-fund/

All donations will be receipted by Ottawa Community Foundation.

The changes to our delivery model was made after much consultation and discussion. This new model reduces expenses while continuing to support pallaitive care in the Champlain Health Region.

We have an important announcement.

As of December 31, 2023, the OutCare Foundation will cease be a registered charity and will transition to the OutCare Fund at the Ottawa Community Foundation.

As the world started to emerge from the COVID era, we had an epiphany – OutCare did not need a physical office or the associated administration costs. We realized we could continue to support area hospices with significantly lower overhead costs.

The Diane Morrison Hospice at is our final grant recipient for fall 2023, and we are so excited to announce they will be using this grant to purchase a bariatric bed!

The Ottawa Mission has been a staple in its community since 1906, and this bed will provide the Diane Morrison Hospice with the ability to enhance care.

We are excited to announce Home Hospice North Lanark as our second recipient of our fall 2023 grants!

provides services for many communities in North Lanark, as well as caregiver support and resources.

This grant will help these services reach more people who will benefit from these incredible services.

We are thrilled to announce our first grant recipient, Beth Donovan Hospice!

Beth Donovan Hospice has been providing services to their community for over 30 years, and will be using this grant to continue their incredible work.

Their 7-week speaker series, “When the news is not good" will discuss topics including:
- Navigating the health system and how to advocate for yourself
- Myth vs Facts about naturopathic options
- Difficult conversations with loved ones
- Discussions about end-of-life care and end-of-life planning
