Prescott Family Health Team (PFHT) is a group of health care professionals working collaboratively to provide comprehensive and accessible health care.

Services are provided by a intradisciplinary team including administration, reception, dietitian, nurses, nurse practitioner, physicians, and a social worker, Nurse Practitioner and Physician Assistant. Patients and families have access to an internist and psychiatrist; these services are fully integrated at the Prescott clinic.

📣 The Prescott Family Health Team is currently seeking a Temporary Full-Time Registered Practical Nurse (RPN) to join our interdisciplinary team to provide maternity leave coverage for an 18-month term commencing August 4, 2026.

PFHT is a collaborative group of healthcare professionals providing comprehensive and accessible care in Prescott. Our intradisciplinary team offers a supportive and professional environment focused on community health.

What an incredible day! 👏🥳

At Prescott Family Health Team, investing in our team’s well-being continues to be a top priority.

Our 4th Annual Employee Health and Wellness Event on April 15th was a huge success—filled with laughter, meaningful conversations, and opportunities for growth and connection.

This year, we were thrilled to welcome Kim M and Abby McIntyre as our guest speakers. They shared valuable insights and fresh perspectives, helping us strengthen communication, build resilience, and deepen our sense of teamwork.

Our day was divided into two valuable sessions. In the first, we focused on strengthening communication, teamwork, and resilience by connecting more intentionally with one another. The second half of the day shifted toward conscious leadership, where we explored our working genius and how to better leverage each other’s strengths.

Through collaboration and shared learning, our team continues to grow stronger—supporting one another and working together to provide the highest quality care for our patients.

Early detection can save lives 💗
